Coming to the office with a baby can be stressful. Filling out forms when you’re in the office is even more stressful. To make it easier on you (and me), it is very important that you do BOTH steps below:
Step 1: Fill out your online questionnaire and patient information. When you schedule with me in the office, you will be given access to register through an online portal. You can access that by clicking here.
Step 2: There will be a lot of information presented at your visit. I’ve heard that sometimes, babies don’t let their parents sleep (just a rumor). Remembering everything can be difficult, so it’s very important that you read the three sections below.
